Birth to Three Program Information


We are a prevention initiative program which focuses on families who are pregnant or have children ages birth to 3 years. A Parents As Teachers Parent Educator is available to support families during the first three years of their child's life. The Parent Educator will do weekly home visits as well as help connect families to community resources that fit the individual needs of the family. We serve families in the Unity Point School District.

ALL CHILDREN ENROLLED IN THE UNITY POINT BIRTH-TO-THREE PROGRAM WILL NEED THE FOLLOWING BEFORE THEY CAN PARTICIPATE:
  • Certified copy of birth certificate or passport at the time of your child's screening
  • A physical completed within the past year and up to date childhood immunization record within 30 days of enrollment
  • All enrollment forms completed
  • Income verification
